They use an empty building which initially was a quarters in the KK itself for morning exercise 3 times weekly, i.e. Sunday, Tuesday and Thursday.<br /><br />They have video player in which they follow for aerobics. There are over 90 old people, with age ranging from 50+ and above who registered but only 30 are regular attendants. We really enjoyed spending time with these old people, they are sooo friendly and nice. There are 2 officers in charge of this unit for this district, so Mr. Wan has to travel to various clinics on different days to counsel patients regarding food nutrition.<br /><br />His patients are generally antenatal mothers who are anemic and requires Iron supplements, children who are underweight, obesity problems, Thyroid disorders, and adolescents problems like hypertension, Diabetes Mellitus, obesity. His job is mainly to counsel them on healthy eating habits apart from the medications they may be taking.<br /><br />In the community, programmes like health talks on weight management, balanced diet, healthy eating habits for senior citizens, children, adolescents and Diabetics are also carried out. Next programmes like Rancangan Bakul Makanan (Food baskets programme), Penyusuan susu ibu (breast feeding programme) and Iodine deficiency disorders are those that are commonly done in this district, mainly by Mr. Wan himself.<br /><br />When we talk about malnutrition in children, we have to probe the questions to the parents in a few aspects. E.g. who is taking care of that child? when did they start to wean? What's the pattern of weaning? e.g. after 6 months of age, are they only given a little bit of semi solids but milk of a bigger portion or they do not take semi solids at all but only drinks milk, when the child's energy requirement increases due to more physical activities? All this must be looked into, however Mr. Wan told us this condition is commonly due to poverty.<br /><br />The food basket programme that was mentioned earlier, is created for those with per capita income of RM 112-115 and weight that falls into the yellow/red centile. (Refer to the follow up card of a child, the blue and red cards in KKIA) and for those of 6 months to 6 years of age.<br /><br />The food basket contains food of all classes, i.e. rice, beehoon (Carbohydrates), ikan bilis, Sardines and Eggs (Protein), cooking oil, margerine (Fats), full cream milk 1kg per month, multivitamins. Only dry food are given and given according to the age of the child, but an extra 50% of the normal calories requirement for the child are given. This basket is estimated to be about RM 150.<br /><br />Their weight and height will be assessed by the MO every 4 months and sometimes the nurses will travel to some houses for demonstration of cooking. They will be given this food basket until their weight returns to normal (the white centile in the growth chart) or when they start going to school at 7 years of age, and if they are still underweight at that time, they are eligible for RMT (Rancangan Makanan Tambahan).<br /><br />There are currently 13 malnutrition cases in Kuala Muda district. So if the owner fails to fulfill the first column there with 15 demerit marks, he'll be left with 88-15=73. Sultan Abdul Halim (HSAH)An extra activity by the Law and Enforcement Unit on a bright and hotttt Saturday afternoon in which an operation of issuing compounds/summons to those caught smoking in prohibited areas, and this time the operation is in HSAH. We're very lucky to be able to join because this only occurs twice or thrice in a month.<br /><br />We gathered at HSAH at 230p.m.. The officer in charge is Mr. Adam Abu Bakar from the Unit Penguatkuasaan (Law Enforcement Unit). A very helpful officer who shared very useful information with us. 9 of us are divided into 3 groups, as there are a few other volunteers from HIV/AIDS Unit, Vector Unit and officers from BAKAS helped out too. Mr. Adam said that this is because the Law Enforcement Unit is lack of staff and because they are dealing with people who smoke in prohibited areas, they will need more people in a team to help "arrest" the person red-handed (i.e. with a cigarette that is still LIGHTING and that they have it in their mouths.), because there were cases when the person who was caught was unhappy and he tried to beat up the officer in charge that day and it became a police case. I would say it was more like a motivational talk to the villagers and to educate them regarding needle sharing and encourage them to be devoted to God so that they can stop their addictions for commonly Heroin and Morphine.<br /><br />The factors which cause people to start "trying" drugs (dadah):<br /><br />56.66% are due to peer influence, 18.73% are curious, 15.32% thinks that it is for fun, 5.44% due to mental stress, 1.42% are due to stimulus (rangsangan), 0.76% to overcome pain, 0.54% was not aware of it and others (no reason) takes up 1.24%. She spoke on the treatment and side effects of drug addiction.<br /><br />She mentioned about the Methadone Replacement Therapy in which this is used as a substitute for Morphine and Heroin and is used to reduce the addiction to them. It can only be prescribed first by a Medical Officer and then subsequently by a Pharmacy Officer. It will take effect after 1 hour, peaks at 4 to 8 hours. It is taken once a day for A YEAR. However, the success rate is only 1 out of 100.<br /><br />She also mentioned that it requires a lot of disciplince and family support to really keep to this therapy. They can be healthy by losing weight and prevent some illnesses like Artherosclerosis, Cardiac diseases and obesity causing Diabetes Mellitus.<br /><br />The 2 activities which are also carried out by UPK:<br /><br />Program Doktor Muda (Young doctors programme) - currently over 28 primary schools in the Kuala Muda district have the Doktor Muda club, where students of Standard 2, 5 and 6 are selected to join based on their academic performance. 30 students with good results will be selected from a school and they have a list of activities on e.g. first aid techniques which would expose them to ways of handling an injury and etc.. Before year 2009, the officers from UPK will visit schools from time to time to follow up on the activities of the Doktor Muda club and also teach and brief them regarding various health issues based on a module they follow. After year 2009, the teachers in charge will give the briefing to the students after being taught by the officers from UPK. Under this programme, a camp called PROSTAR Facilitator Camp is done by selecting a few adolescent students to participate in this camp, and later they serve as a facilitator to teach their peers about what they have learnt in this camp. Then the PKA will go to the respective areas and see which area needs it more because the quota is limited. The government gives RM 800 for an approved application of toilet. It operates like a septic tank but this the "rural area version" of septic tank. (To lower the number of new Dengue cases in Kuala Muda district) They pay more attention to Dengue cases because it's of the highest incidence; however the same objective applys to all other vector borne diseases like Malaria and Filariasis.</li><li>Mengelakkan berlakunya wabak tidak terkawal (To prevent outbreaks)</li></ul><br />The 3 common indices used are: Aedes index, Brateau index and Container index. E.g. high grade fever, myalgia, body ache, Rashes, retro-orbital pain.<br /><br />Just for your information, as of 29/6/2010, there are 166 Dengue cases reported in Kuala Muda and 112 cases are tested positive. In year 2009, there are 272 Dengue cases which were reported and this year, they aim to reduce this number by 10%. However, there are difficulties in achieving this. E.g. when the DHO send their staff for fogging in a Dengue epidemic area, 100% of coverage cannot be achieved because some houses are not occupied and some are locked during the time of fogging, and they are not allowed to enter a house if there's no residents or the residents who are at home at that moment are under 18 years old. Mr. Rosli also told us that it is rather difficult to educate the people to prevent the breeding of Aedes mosquitoes.<br /><br />It was also reported that there are over 30 schools in this district which have breeding grounds for the Aedes species!<br /><br />Health talks, campaigns, and even massive cleaning (gotong royong) are some of the activities which are carried out by this unit as a measure of educating and preventing vector borne diseases especially Dengue.<br /><br />At about 11a.m., we drove to Taman Keladi (near Metro Hospital) which there was a Dengue positive case reported in the past 1 week. The officers were divided into 2 teams, 2 in each where they go house by house for Aedes survey. The questions they asked include the name of the owner of the house, and anybody who recently had fever in that house. The officers also check their bathroom (for stagnant water in the bath-tub-like thing, you guys know that one cement walled "pond" where they store water for bathing purposes) for mosquito larvae. It is a water-based formula which does not leave the oily and sticky stuff in the house, which is not understood by many. A lot of people think that they are using an oil based formula and many residents have refused fogging in the interior of the house. E.Coli commonly used as an indicator in these sites.<br /><br />Disease Surveillance - enables us to fortell or predict occurence of outbreaks, as in the case of dengue, an Aedes Index is used: no of vector breeding sites/no of houses surveyed in given area in percentage enables us to determine density of vector. With adequate information, the government is better able to take certain measures.<br /><br />Food Quality and Control - prohibiting entry of imported products that do not comply to safety standards outlined by the Food Act. and Collaborative Operations carried out regularly by Health Inspectors and Law Enforcement Officers ensures that consumer rights are preserved.<br /><br />Health Education Unit - speech, exhibits in schools and competitions held regularly to promote awareness.<br /><br />etc.<br /><br /><br /></span><span style="font-family:times new roman;font-size:100%;">2.
